For more than ten years, Dr. Meredith Sweeney was a board-certified bariatric surgeon at Norton Healthcare in Louisville. She performed the operations that treat the consequences of metabolic disease after every other intervention has failed. Most of her surgical patients had tried diets, primary care, and online prescribers before they ended up on her operating table.
“In the operating room I would see the ravaging effects of metabolic dysfunction on the body. And what’s sad is that with proper education and proactive medical care we could have avoided ending up here.”
By her own count, Dr. Sweeney spent 90% of her time outside the operating room managing the lifestyle and metabolic health of her bariatric patients. She rewrote the preoperative and postoperative diet protocols at Norton because nobody else was going to do it. The Weight Loss Program is the practice she built after more than a decade watching what happens when people try to lose weight without the data to do it correctly.
“I am a fixer, I always have been, which is why I went into surgery in the first place. At Sano Longevity I still help patients fix their health, I just do it from the ground up.”
Baseline data captured. CGM protocol runs. First interventions introduced. Body composition starts shifting by week 8 to 12.
The protocol compounds. Fat loss accelerates. Visceral fat drops in measurable square centimeters. The metabolic shifts in this window are the difference between weight loss that holds and weight loss that comes back.

Every tier includes the full foundation. The differences are in the depth of coaching, dietitian, and retesting support.
First visit fasted, in-office. VO2 max test, InBody 580 body composition, resting metabolic rate on the PNOĒ platform, 46-marker longevity lab panel via Quest Diagnostics. Approximately 2 hours total.
Approximately one month after the first visit, Dr. Sweeney walks you through every result and presents your individualized weight loss roadmap.

You complete the Sano intake questionnaire. More in-depth than anything you have seen from a physician.
Fasted and in-office. VO2 max test. InBody 580 body composition scan. Resting metabolic rate on PNOĒ. Quest blood draw. Total on-site: approximately 90 minutes.
Dr. Sweeney walks you through every result for 60 to 90 minutes. GLP-1 prescribed if data supports it. CGM started. Nutrition and training plan delivered.
90-day CGM with weekly assignments. Body composition retesting on demand. First measurable fat loss visible on InBody by week 8 to 12.
Fat loss accelerates. Visceral fat drops in measurable square centimeters. Muscle preserved because the protocol is built around it.
Dr. Sweeney walks you through everything that moved. Patients who continue can move into a maintenance phase, with or without GLP-1, based on what the data says.
You worked your way up to the maximum dose on a Hims, Hers, or telehealth GLP-1 prescription. You had the side effects. You did not have the results. The reason is that the medication alone is not the protocol. The Weight Loss Program gives you the body composition data, the protein target, the training zones, and the physician oversight that the chatbot version did not.
Your body changed and the things that used to work do not work anymore. You want to feel great in your own skin. The Weight Loss Program measures the metabolic shifts of perimenopause and builds a protocol around them, with or without medication, depending on what your data says.
You have been on a scale your whole life. You want a real measurement of where your fat is, how much of it is the dangerous kind (visceral fat), and whether the weight you are losing is fat or muscle. The Weight Loss Program gives you that measurement at every visit, on the same InBody 580, with the same physician.
She is a 41-year-old business owner, mom of two, perimenopausal. She walked into Sano Longevity dealing with a body that was changing and feeling out of her control. She said: “I just want to feel great in my own skin.”
Sano started her on a very low dose of GLP-1. She wore a continuous glucose monitor for 90 days. Her nutrition, exercise, and supplement plan were built on her actual metabolic data and her actual glucose curves, not on a formula. Dr. Sweeney walked her through a structured plan to wean off the medication over several months.
Six months later:
Down 12 lbs total | 10 lbs of pure fat (only 2 lbs of muscle loss)
Body fat percentage: from 22% to 17%
Visceral fat down 10 sq cm
Successfully weaned off GLP-1, results held
The result that matters is the last one. The medication was a tool. The protocol around it is what made the results stick. She is feeling great in her own skin.
Seven questions patients ask most often about the Sano Weight Loss Program.
Sano Longevity restructured the Weight Loss Program around a 12-month commitment because the data is unambiguous: long-term accountability is the difference. Patients who commit to long-term accountability are up to 80% more successful in keeping the weight off. The 12-month timeline produces the body composition shifts that take a quarter or more to register on an InBody scan, and the metabolic marker improvements that take six months or longer to move.
The Weight Loss Program includes a full first-visit assessment (VO2 max, resting metabolic rate, InBody 580 body composition, 46-marker longevity lab panel through Quest Diagnostics), a 90-day continuous glucose monitor protocol with weekly assignments, a personalized nutrition, exercise, and supplement plan, quarterly physician appointments with Dr. Sweeney, an annual lab panel, unlimited body composition scans, concierge access, 20% off IV and IM therapies, and physician-supervised GLP-1 prescribing when clinically appropriate. Two tiers: Gold and Elite.
Online GLP-1 prescribers issue prescriptions from a chatbot intake and a video call. They do not measure body composition, visceral fat, metabolic rate, VO2 max, or fasting insulin before prescribing. They do not build a muscle preservation protocol. They do not monitor whether the weight you are losing is fat or muscle. Sano Longevity runs a full metabolic and body composition workup before prescribing, builds a muscle preservation protocol around the medication, and tracks fat-only weight loss with InBody 580 retesting at every visit.
A muscle preservation protocol is a structured combination of protein prescription (calculated from your resting metabolic rate, not estimated from a formula), strength training programming built around your VO2 max-derived training zones, body composition monitoring at every visit, and physician oversight of medication dosing relative to muscle loss. Without this, GLP-1 weight loss often comes 30 to 40% from muscle, which permanently lowers metabolic rate and increases the risk of weight regain.
Some Sano patients stay on a maintenance dose of GLP-1 indefinitely. Others wean off completely once their body composition, fasting insulin, and metabolic markers have stabilized. One Sano patient, a 41-year-old perimenopausal business owner, lost 12 lbs (10 of pure fat), dropped her body fat from 22% to 17%, weaned off GLP-1, and held her results. The decision to wean off is made between patient and physician based on the data, not on a timeline.
Sano Longevity is a membership-based practice. Insurance is not accepted. The practice is HSA and FSA eligible. This model exists so Dr. Sweeney can spend 60 to 90 minutes with each patient reviewing data, instead of the 12 minutes a typical insurance-billed visit allows.
The Sano Weight Loss Program is membership-based with two tiers. Gold is $349 per month and Elite is $499 per month, plus a one-time $350 onboarding fee. Both tiers include the comprehensive 46-marker longevity lab panel, body composition testing on the InBody 580, GLP-1 medication prescribing and monitoring when clinically indicated, dietitian consultations, and physician appointments with Dr. Sweeney. Membership requires a 12-month minimum commitment. HSA and FSA eligible.
