Solo physician practice. Tightly held team. The same people across every visit. No rotating providers, no interchangeable clinicians, no losing track of your story between appointments.
30-minute call. No obligation. Cash-pay practice, HSA and FSA eligible.
Reviewed by Dr. Meredith Sweeney, MD, FACS, FASMBS | Board-certified bariatric surgeon, A4M member, founder of Sano Longevity in Middletown. Last updated April 2026.
Sano Longevity is a solo physician practice. Dr. Meredith Sweeney is the only physician in the practice and is the same physician you will see at every appointment. The supporting team (Emily Coles RD, Melissa, Kristen Cosinek, Dr. Julie Sargent DPT through Sargent Wellness) is small and tightly held by design.
The reason matters. Longevity medicine is multi-quarter work. The physician who sees you at month 12 needs to remember your month 1 data, your month 4 conversation about sleep, and your month 8 lab shift. A practice with rotating providers cannot deliver this kind of continuity. A solo practice can. The trade-off is that Sano is not built for high-volume same-day access. The benefit is that the patient who starts with Dr. Sweeney finishes with Dr. Sweeney, every time.
If she is out for two weeks, the clinic closes. That is a feature, not a bug.
Founder, Sano Longevity. Board-certified bariatric surgeon turned longevity physician. The only physician in the practice. The same physician you will see at every visit.
Dr. Meredith Sweeney spent more than ten years as a board-certified bariatric surgeon at Norton Healthcare in Louisville before founding Sano Longevity in 2025. By her own count, she spent 90% of her time outside the operating room managing the lifestyle and metabolic health of her bariatric patients, filling a gap the system was not filling. She rewrote the preoperative and postoperative diet protocols at Norton with no formal nutrition credential, because nobody else was going to do it and her patients needed it.
I’m a fixer. I start at the bottom and fix it there, and then everything upstream is magically better.
Dr. Meredith Sweeney
She is the only longevity physician in Louisville with a decade of experience operating on the consequences of metabolic disease. She is currently pursuing American Menopause Society certification and is a regular contributor to longevity medicine podcasts and conferences. See membership pricing · Read Dr. Sweeney’s full story.
How to work with her: Dr. Sweeney is the physician at every clinical appointment. All patients work with her directly from the roadmap appointment onward.
Registered dietitian. Available to Longevity Track Executive tier members for structured nutrition sessions in the first year of membership.
Emily Coles is a registered dietitian who works virtually with Sano Longevity Executive tier members from her base in South Carolina. She partners with Dr. Sweeney to translate the data from each patient’s PNOĒ Metabolic Blueprint, InBody 580 body composition analysis, and 25-marker metabolic blood panel into a practical, patient-specific nutrition plan. She is available to Executive tier members on the Longevity Track and the Weight Loss Track for structured sessions during the first year of membership.
How to work with her: Emily’s sessions are included automatically for Executive tier members. Patients on the Gold or Platinum tiers can ask Dr. Sweeney about adding dietitian sessions on a per-session basis.
Longevity coach and life coach. Works with Executive tier members on mindset, motivation, habit change, and the implementation gap between knowing what to do and actually doing it.
Last name pending team approval for publication.
Melissa is the longevity coach on the Sano team. Her job is to help patients close the gap between the longevity plan Dr. Sweeney builds and the day-to-day execution that turns the plan into results. Most patients who walk into Sano already know they should eat better, sleep more, exercise consistently, and manage their stress. The reason they have not is rarely a lack of information. It is the dozens of small mindset, habit, and motivation barriers that get in the way of consistent execution. Melissa works with patients on those barriers directly.
How to work with her: Melissa’s sessions are included automatically for Executive tier members on the Longevity Track. She works virtually with patients via scheduled coaching sessions throughout the year.
Doctor of physical therapy. Owns Sargent Wellness, an allied partner facility. Provides annual strength, mobility, and injury screens for Sano Executive tier members.
Dr. Julie Sargent is a doctor of physical therapy and the owner of Sargent Wellness. She is an allied partner of Sano Longevity, not a Sano employee. Sano Executive tier members receive an annual strength, mobility, and injury screen at her facility as part of their membership. The screen includes a grip strength measurement (one of the strongest predictors of all-cause mortality in adults over 50), a flexibility and range-of-motion assessment, and an injury screen that catches the asymmetries and imbalances that lead to injury during a longevity training protocol.
How to work with her: The annual screen is included automatically for Executive tier members. Patients on the Gold or Platinum tiers can ask Dr. Sweeney about adding the Sargent Wellness screen on a per-visit basis.
Patient concierge liaison and personal assistant. Manages patient scheduling, intake coordination, and the day-to-day patient experience. Mostly virtual.
Kristen Cosinek joined Sano Longevity in early 2026 as the patient concierge liaison. She is the first point of contact for new patient inquiries, the person managing the appointment cadence for active members, and the bridge between Dr. Sweeney’s clinical work and the patient experience. She works mostly virtually but can run metabolic testing in-office when needed.
How to work with her: Kristen handles all new patient inquiries. When you book a free consultation through the Sano website, Kristen is the person who reaches out to schedule it.
